If I want to test 5 different image hooks and 3 different headlines on Meta ads, what’s the cleanest way to set up the campaign so budget isn't spread too thin across variants?

It'd be too expensive to try all the 15 combos of this so maybe I’d start with one campaign first and one ad set, and you test two or three images at a time with the same headline, audience, and offer. I would then try testing the strongest image with my three headlines. Based on the data I would then make a judgement call which to keep using moving forward.

I’d probably start with fewer variations instead of testing everything at once. Pick a couple of the strongest hooks and headlines first, see which ones get better results, then use the remaining budget to test new variations based on what you learn. It keeps the testing more manageable without spreading the budget too thin.

Keeping the audience and offer the same while testing the creatives would make things easier to compare. I’d also give each version enough time to collect some data before changing anything, otherwise it can get pretty hard to tell what’s actually working.

One thing that could help is changing just one part of the ad at a time. That way, if one version performs better, you have a clearer idea of what made the difference without having to test a bunch of variations at once.
